Helping People Across Surrey Better Manage Their Diabetes
CSH Surrey Extends Diabetes Education Partnership Following Outstanding Patient Outcomes.
X-PERT Health is pleased to announce the extension of its managed service partnership with CSH Surrey through 2026–2027, following the successful delivery of 50 diabetes education programmes since 2024.
Delivered through a flexible hybrid model combining face-to-face, online and digital learning, the service has supported patients across Surrey to better understand and manage their diabetes through evidence-based education and long-term lifestyle support.
Since launch, the programme has achieved exceptional patient engagement and clinical outcomes, including:
👉 86.7% programme completion rate
👉 95.2% patient evaluation score
👉 16.2% improvement in patient empowerment
👉 Average 4.7kg reduction in weight
👉 Average 13.2 mmol/mol reduction in HbA1c
👉 Reductions in blood pressure and blood lipid medication use
The extension of the contract reflects the programme’s measurable impact on patient confidence, self-management and health outcomes, alongside consistently positive patient feedback across all cohorts.
Patients described the programme as “eye opening”, “invaluable” and “life changing”, with many highlighting the supportive learning environment and practical tools that helped them make sustainable lifestyle changes.
One participant commented:
“The knowledge I gained is invaluable, going forward will be a great help.”
Another said:
“I feel I’m now equipped to approach my diabetes, ongoing diet and life with more tools than I previously had.”
Participants also praised the programme facilitators for their supportive and engaging delivery style, with one attendee stating:
“An excellent course run by a brilliant lecturer. Friendly, helpful, caring and a true professional.”
The programme’s hybrid delivery model has played a key role in increasing accessibility and supporting diverse learning needs across Surrey communities. Combining structured education, interactive sessions and digital support has enabled participants to engage in a way that works best for them.
