NICE Type 2 Diabetes Guidelines Updated — But Significant Issues Remain

The latest update to the NICE Type 2 Diabetes management guidelines (NG28) has now been published. While this update had been anticipated as an opportunity to address long-standing concerns around lifestyle advice and patient choice, the final guidance falls short of expectations and leaves major issues unresolved.

Over the past year, X-PERT Health, alongside partner organisations, has been actively engaging with NICE to highlight critical limitations in the existing guidelines. These include the lack of support for type 2 diabetes remission as a realistic goal, insufficient recognition of different evidence-based dietary approaches, and a failure to adequately prioritise informed, patient-centred choice.

Earlier correspondence from NICE had suggested that these concerns would be considered as part of an ongoing update to NG28. However, despite stakeholder submissions and detailed feedback on the draft guidance, the final version of the update does not introduce any meaningful changes in relation to these issues. Many of the points raised were deemed “out of scope”, despite their clear relevance to patient care and consistency with other NICE guidelines.
