Structured education is the foundation of effective long-term condition management
For more than 20 years, X-PERT Health has been helping people develop the knowledge, confidence and practical skills needed to manage their health more effectively, supporting better outcomes and more sustainable behaviour change.
Today, more than 500,000 people have participated in X-PERT programmes delivered by over 600 trained educators across the UK. Through a combination of evidence-based education, self-management support and practical lifestyle guidance, our programmes empower people to take an active role in their health while supporting more efficient use of healthcare resources.
The effectiveness of X-PERT programmes is supported by a robust evidence base, including randomised controlled trials, national programme audits, NICE-compliant structured education frameworks, published peer-reviewed research and real-world implementation across NHS organisations.
Key Findings from the X-PERT Audit 2025
The latest audit data demonstrates the measurable impact structured education can have on both clinical and behavioural outcomes. Participants achieved an average HbA1c reduction of 9.3 mmol/mol within six months, while almost half (46.5%) reduced or omitted diabetes medication during the same period. This included 472 people who stopped medication entirely, representing an estimated annual saving of £236,944 for the NHS.
Improvements were also observed across a range of important health indicators, including weight, BMI, waist circumference, blood pressure and cholesterol ratios. Importantly, the programmes continue to reach diverse populations, with 30% of participants coming from ethnic minority communities, significantly higher than national census proportions.
More Than Clinical Outcomes
The value of structured education extends beyond clinical measures alone. Participants consistently report feeling more knowledgeable about their condition, more confident in managing their health and better equipped to make informed decisions about their lifestyle and treatment options. Recent audits demonstrated an approximately 20% improvement in participant empowerment scores, highlighting the important role education plays in building long-term self-management capability.
Long-term health outcomes are rarely achieved through a single intervention. Increasingly, healthcare systems recognise that sustainable change requires a combination of clinical treatment, practical education and ongoing support. X-PERT programmes complement a wide range of health and wellbeing services, including prevention programmes, diabetes care, weight management pathways, GLP-1 services, workforce wellbeing initiatives and community health programmes.
By helping people understand their condition, build confidence and develop practical self-management skills, structured education provides the foundation for lasting behaviour change and improved health outcomes.
Participants achieved an average HbA1c reduction of approximately 10 mmol/mol within six months, with more than 46% reducing or omitting diabetes medication. Improvements were also seen across weight, blood pressure and cardiovascular health markers.
Audit data demonstrates average weight loss of 4.5–4.7kg at 12 months, alongside improved eating behaviours, increased confidence and sustainable lifestyle changes.
