Diabetes Support in Nottinghamshire for the South Asian Community

Boots Charitable Trust has awarded X-PERT Health a special grant to deliver:
• Digital diabetes education for 100 South Asian individuals living in Nottinghamshire
• Available in Urdu, Bengali, Arabic, Gujarati, Hindi, Punjabi, or Tamil
• Plus 2 hours of one-to-one health coaching in their preferred language
Who Can Join?
✔️South Asian community members in Nottinghamshire
✔️Diagnosed with or at risk of Type 2 diabetes
✔️Age 18+
✔️Have access to a smartphone, tablet, or computer with internet
What Participants Receive
• Interactive digital learning platform with animated videos, quizzes, and games
• Personal health coaching in their native language
• Comprehensive 165-page Diabetes Handbook (English)
• Specialised 18-page handbook in their chosen language
• Access to peer support through group chat features
Why This Matters
The stark reality: Among 68,555 people with Type 2 diabetes in Nottinghamshire, 6,000 are South Asian. This community faces higher risks yet lower participation in traditional programs. Our tailored approach directly addresses cultural and language barriers that prevent access to life-changing education.

• Blood glucose control (HbA1c)
• Weight management
• Blood pressure levels
